Are You the Parent of a 6th Grader or Pre-Schooler?
State law requires students in these grades have certain immunizations (Diptheria, Tetanus, Pertussis (Tdap) and Meningococcal, depending on grade).
See the letter and immunization update form you can bring with you to your doctor.
Please note that it must be returned to your 天美视频 nurse by Oct. 14, 2022 or your child will not be allowed to attend 天美视频 after that date.
